Job Summary
The Mobile Crisis Clinician rapidly responds 24/7 to requests for
crisis interventions in the community. Referral sources include
health care providers, family members, self-referral, police, and
other community members. Performs telephone triage, mobile crisis
assessments, risk assessments, de-escalation, and safety planning.
- Works with Emergency Medical Services facilitating referrals to
emergency department, community mental/substance abuse treatment
and other linkages in the community. The Mobile Crisis Clinician
covers the towns of Meriden and Wallingford and serves individuals
18+ years of age.
This role is considered an essential employee and is required to
provide coverage of holidays on a rotating basis and may be
required to stay for mandatory shift coverage. May respond to calls
with other community partners including but not limited to police,
fire, EMT, health department.
This position is for our mobile crisis team covering Meriden and
Wallingford. Position is second shift with flexibility to modify
schedule to include first or second shift on weekends based on
candidates availability for 40 hours per week. Candidate should
have experience working in crisis assessment, able to work
independently, and have sound clinical decision making skills.
Shift differential eligible position.
Job Responsibilities
Provides comprehensive initial and ongoing assessment of
individuals in crisis in the community with special focus on risk
assessment, mental status exams, safety planning and both personal
and community safety needs. -
Functions as a collaborative member of the Crisis Team to ensure
optimal, recovery-oriented care to both mental health and
co-occurring individuals through a variety of treatment services,
including but not limited to and crisis assessment, management, and
crisis stabilization of individuals in the community as well as
daily clinical consultation with staff and clients.
Provides therapeutic and case management support while referring
and linking clients to clinical services. -Provides phone triage
and support to clients in need of crisis services or ongoing phone
consultation.
Serves as a liaison to local police and emergency services
organizations on crisis services, provides outreach and education
on crisis services, and develops partnerships to effectively
respond to clients in crisis.
Ensures all documentation requirements and billing procedures are
completed and submitted in a timely manner to support accurate and
timely reimbursement of agency services. Ensures that all state,
federal, Joint Commission and other oversight regulations are
strictly adhered to.
Works across programs for clients with both mental health and
substance use disorders.
Develops and maintains recovery- oriented therapeutic relationships
with individuals in crisis through ongoing assessment and
stabilization efforts.
Collaborates with clinical team to develop person-centered
strengths based safety planning.
Participates in administrative activities including, team meetings,
clinical supervision of staff and staff education as designed by
the Program Manager.
Perform other related duties as required or assigned.
Qualifications
Master's Degree in Social Work, or Counseling, or Marriage and
Family Therapy or Related Human Service Field
Three years' providing direct services to psychiatric patients
and/or substance abuse population
Three years' previous experience in assessment and diagnosis and
crisis intervention
LCSW, LPC, LMFT, LADC
Valid Driver's License

Assumes responsibility for self-improvement by identifying,
communicating and seeking resources to meet own learning needs.
Flexible with schedule per program needs, including
evenings/weekends and travel in own vehicle between sites across
the organization and/or community per job responsibility
requirements.
Intermediate experience with Microsoft Office products including
Word and Outlook.
Experience in or the ability to learn how to use an Electronic
Medical Record (EMR) system, which includes appointment scheduling,
authorization tracking, assessments, treatment plans, progress
notes, and discharge summaries.
